INCORRECT GRADE BOLTS WERE USED IN ATTACHING THE BODY TO THE CHASSIS AND IN ATTACHING THE SEAT ONTO THE CHASSIS.

Injuries / Consequence

PROPER TORQUE COULD NOT BE ACHIEVED AND BODY COULDSEPARATE FROM THE CHASSIS DURING AN IMPACT.
