Description

THE THREE-POINT LATCHES USED ON THE EMERGENCY DOORS HAVE BEEN MISTAKENLY ASSEMBLED USING STANDARD NUTS RATHER THAN LOCKING NUTS. THE STANDARD NUTS COULD BACK OFF DUE TO VIBRATION, CAUSING THE RODS WHICH UNLOCK THE EMERGENCY DOORS TO FALL OFF.

Injuries / Consequence

IF THIS OCCURS, AND AN EMERGENCY ARISES, THE EMERGENCYDOOR CANNOT BE OPENED FROM THE OUTSIDE OF THE BUS.

Remedy

THE STANDARD NUTS WILL BE REPLACED WITH LOCKING NUTS.
